PRE-SCREENING FOR COVID-19 SYMPTOMS

  • If you are experiencing cold or flu-like symptoms, please do not come to the office.
  • If you are concerned that you have symptoms or came into contact with an individual who has COVID-19 please contact your primary care physician via phone.
  • 𝐴𝑛𝑦 𝑝𝑎𝑡𝑖𝑒𝑛𝑡𝑠 𝑤ℎ𝑜 ℎ𝑎𝑣𝑒 𝐶𝑂𝑉𝐼𝐷 𝑠𝑦𝑚𝑝𝑡𝑜𝑚𝑠 𝑜𝑟 ℎ𝑎𝑣𝑒 𝑡𝑒𝑠𝑡𝑒𝑑 𝑝𝑜𝑠𝑖𝑡𝑖𝑣𝑒 𝑓𝑜𝑟 𝐶𝑂𝑉𝐼𝐷 𝑖𝑛 𝑡ℎ𝑒 𝑝𝑎𝑠𝑡 14 𝑑𝑎𝑦𝑠 𝑤𝑖𝑙𝑙 𝑛𝑜𝑡 𝑏𝑒 𝑏𝑜𝑜𝑘𝑒𝑑 𝑎𝑡 𝑡ℎ𝑖𝑠 𝑡𝑖𝑚𝑒.
  • Please call ahead if you feel sick, have red eyes (conjunctivitis) or any concerns.
  • We reserve the right to refuse service to anyone.

APPOINTMENTS

𝑀𝑜𝑛𝑑𝑎𝑦 - 𝑆𝑎𝑡𝑢𝑟𝑑𝑎𝑦 𝑓𝑟𝑜𝑚 10𝑎𝑚 -3𝑝𝑚
  • We will be restricting entry into the practice to patients with appointments only. Appointments will be held for 10 minutes, after this grace period we consider this as a “NO SHOW” and you will need to reschedule.​
  • Please limit the number of people accompanying you to our office to 1 person. Absolutely no eating or drinking in the office.
  • To promote social distancing, we will require all patients to make an appointment at https://www.shadesofbrooklyn.com/, or over the telephone prior to their visit. This includes eye wear/contact lens consultations, medical or follow up visits, repairs/adjustments and pick-ups.
  • Demographic, medical history and insurance forms will be available electronically on https://www.shadesofbrooklyn.com/ via our Patient Portal or 𝑐𝑙𝑖𝑐𝑘𝑖𝑛𝑔 𝑜𝑛 𝑃𝑎𝑡𝑖𝑒𝑛𝑡 𝐹𝑜𝑟𝑚𝑠 𝑎𝑛𝑑 𝑠𝑒𝑙𝑒𝑐𝑡𝑖𝑛𝑔 𝑁𝑒𝑤 𝑃𝑎𝑡𝑖𝑒𝑛𝑡 𝐼𝑛𝑓𝑜/𝑀𝑒𝑑𝑖𝑐𝑎𝑙 𝐻𝑖𝑠𝑡𝑜𝑟𝑦, please fill them in before your visit.
  • Paperwork is to be completed by each patient electronically prior to your arrival for your appointment. This is to decrease touch points within the office.
  • Please limit the number of people accompanying you to our office to 1 person. Absolutely no eating or drinking in the office.

𝐕IRTUAL WAITING ROOM

  • We will be implementing our virtual waiting room. When a patient arrives they are to call or text us and remain in their car or outside until we are ready to start their exam.

PROMOTE SOCIAL DISTANCING 2M=6FT

  • To promote physical distancing, we will also require patients to enter the office alone unless they are of minor age, require a medical decision maker or require physical assistance.

MASKS FOR SAFETY

  • When you schedule an appointment, you will be required to bring your own mask or bandana. If you do not have a mask or face covering, you may have to reschedule your appointment. Everyone, including those accompanying the patient, must wear a face mask/covering while in the office.
  • All staff will wear face shields or masks for your protection.

FREQUENT DISINFECTION & DEEP CLEANING

  • You will be asked to touch as few surfaces as possible during your visit. All surfaces will be disinfected with high-level cleaners between each patient visit.
  • We are also limiting the number of appointments per day to provide enough time for cleaning in between each patient.
